What is the DiGAs framework and how does it impact digital health and healthtech companies? The DIGA (Digital Health Applications) framework in Germany is an innovative reimbursement system for digital therapeutics, which was launched as part of the Digital Healthcare Act in 2019. This system enables approved digital health apps to be reimbursed by all statutory health insurance providers in Germany, making them accessible to 90% of the population. Once a digital therapeutic is accepted into the DIGA system, doctors can prescribe it for patients with specific diagnoses, or patients can request it directly from their insurance. The standard coverage is for 90 days, though apps can be re-prescribed as needed. The process for app approval involves a fast-track system that typically takes around three months. Apps may be provisionally listed based on early indications of efficacy, but within 12 months, companies must provide sufficient evidence to gain permanent inclusion. Most apps achieve this through randomized controlled trials (RCTs). Pricing for DIGA apps is set by the manufacturer for the first year, after which it is negotiated between the app developer and the health insurance providers. The system has helped streamline access to digital therapeutics in Germany, serving as a model for other countries. This framework presents a unique opportunity for digital health innovators, but it also requires careful attention to regulatory requirements, clinical evidence, and pricing strategies.
