Thank you Sigrid Berge van Rooijen, for your insightful post. On the whole, I feel, all are valid and useful observations. However I would caution us not to overemphasise the interoperability puzzle. Or at least not to the point where the drive to standardize data formats becomes all consuming, as there are at least two immediate issues that I see with that: First off, when looking at what healthcare data is collected in different places in the healthcare system, especially across borders, there are significant differences, not only in coding formats and data structures, be very much also in what is being recorded and what not. As a result, any attempt to standardise will require decisions on how to reflect such differences in the absence or presence of codes, for the range of different healthcare interventions and -observations. An approach whereby the standardised dataset specification is based on the "lowest common denominator" would result in significant loss of resolution / detail, and thus reduce the potential for deriving valuable insights from that common dataset. Secondly, medical standards, -practices and -guidelines are not the same. For example, in one country alternative medicine such as homeopathy may be fully integrated in the medical practice. In others it may not. The implications of such differences extends beyond just the issue outlined in my first caution. It means that, even when harmonised fields are available for it, the absence or presence of data cannot be attributed the same meaning across different healthcare systems or settings. And conclusions derived from on one set of data, are not valid by definition, for the matching common data set from another setting. Both points contribute to why AI adoption in healthcare is so difficult, as they have significant implications for the ability to scale up and scale out AI solutions. After predicting that AI will help cure cancer, OpenAI founder Sam Altman is taking it a step further by saying AI will eventually replace principal investigators. While I also believe AI will transform drug discovery and contribute meaningfully to eradicating cancer and other diseases, I don’t think cancer researchers need to start updating their LinkedIn profiles just yet.  I agree with Shantanu Singh, a machine learning biologist and co-leader of the Carpenter-Singh Lab at the Broad Institute, who tells STAT that there’s one skill ChatGPT and other large language models haven’t mastered: creativity. It’s this fundamentally human skill that Singh and others believe will keep principal investigators at the center of drug discovery, with gen AI just one powerful tool in their toolbox.
