A huge thanks to the Alberta Virtual Care Coordinating Body (https://lnkd.in/d-dMQzjb) for their insightful work in identifying the entrenched challenges in Canadian healthcare's data infrastructure through their report, Data Disarray (https://lnkd.in/dYDTH9SV). Their analysis highlights what many of us have witnessed firsthand: health information governance in Canada is frustratingly fragmented, undermining the essential goals of portability, accessibility, and truly patient-centered care. This report is a crucial step in rethinking how we govern health data. It’s never just about the technology—it’s about building systems that genuinely serve patients throughout their wellness journey - staying healthy, getting better, and managing their conditions. AVCCB’s work is both a wake-up call and a roadmap, pointing us toward a more unified, patient-first approach to digital health. Bridging these gaps will be challenging, but insights like these make me optimistic about the future of interoperable, patient-centered health information systems in Canada. This is an excellent overview article describing the Canadian Association of Radiologists, Health AI Validation Network (HAIVN). The integration of #AI in Canadian health care is inevitable. How we govern the integration of #AI, by who and when is the question. One key reason why healthcare technologies like AI, telemedicine, and software haven't structurally lowered costs or broadened access for the entire system after decades of experimentation (excluding pure financial engineering of payer contracts) is the limited reimagining of care delivery itself, with asynchronous telemedicine as a notable exception. In the same way that digitizing physical paper failed to exploit the capabilities of a new computing medium for word processing, hosting physician-patient conversations over video does not transform the actual delivery of care. In telemedicine, many of the inputs (e.g., clinician time) and therefore cost structures of in-person care persist, which is why we often see fee-for-service telemedicine companies struggle to achieve economies of scale (i.e., growing revenues non-linearly while growing costs linearly). Until we deconstruct the components of a medical appointment – from intake and triage, to chart review, history gathering, physical examination, diagnosis, treatment, and follow-up – we will continue to be underwhelmed by the disconnect between the transformative potential of these technologies and real-world patient/system impact. I’m optimistic that the emergence of reliable and performant medical AI agents will urge us to perform this deconstruction and redesign the fundamental building blocks of a “medical appointment.”

In today's digital age, the healthcare industry, including pharmacies, increasingly relies on technology to manage patient data, streamline operations, and improve service delivery. However, this reliance on digital systems also brings significant cybersecurity risks that cannot be ignored. Recent cybersecurity incidents in pharmacies across Canada and the US have underscored the urgent need for robust cybersecurity measures. For instance, London Drugs, a major pharmacy chain in Canada, recently experienced a cyberattack that may have put sensitive customer information at risk. This incident highlights the vulnerabilities in their digital infrastructure and the pressing need for enhanced cybersecurity protocols​​. Similarly, in the US, Change Healthcare, a subsidiary of UnitedHealth, fell victim to a cyberattack earlier this year that compromised patient data and disrupted services. To stem the damage, they paid $22M in bitcoin to the ransomware hackers. However, compromised data, such as protected health information or personally identifiable information, allegedly ended up on the dark web. This breach further illustrates the far-reaching consequences of such attacks on the healthcare sector​​. These incidents are a stark reminder of the potential consequences of inadequate cybersecurity in the pharmacy sector. Protecting patient information is not just a legal and regulatory requirement; it's a fundamental aspect of maintaining patient trust and ensuring the continuity of care. To safeguard against these threats, pharmacies must: - Implement Strong Security Protocols: Regularly update software and systems, use encryption for sensitive data, and establish multi-factor authentication for accessing systems. - Conduct Regular Security Audits: Identify and address vulnerabilities through routine security assessments and penetration testing. - Educate Staff: Provide ongoing training on cybersecurity best practices and the importance of vigilance in preventing cyber threats. - Develop a Response Plan: Establish a robust incident response plan to quickly and effectively address security breaches. EHR benefit realization is within reach for healthcare providers with the right evaluation framework. According to the May print edition of Canadian Healthcare Technology, the Clinical and Systems Transformation (CST) project in British Columbia is a leading example of this. With its clinical information system implemented at over 46 healthcare sites, serving more than 42,000 users, the project is one of the largest Oracle Health programs in North America in terms of scope. This article from MaRS on Innovations in Healthtech in Canada highlights that "Despite 93% of primary care physicians using electronic medical records (EMRs), fewer than 40% can share clinical updates digitally." https://lnkd.in/gcksgzq7 And let me start by saying big ups to Rishi Nayyar and the team at PocketHealth for pushing the boundaries on patient access to medical data, especially in Canada. The article rightfully highlights their work as a great example of how technology can empower patients by giving them control over their own health information. That said, there’s a major gap in Canadian healthcare—interoperability. We’re lagging when it comes to enabling healthcare providers to share data across systems, sometimes even across the street. Frameworks like TEFCA in the U.S. are miles/kilometres ahead, providing clearer standards for data exchange between providers. TEFCA and similar frameworks, aren't without problems, but we need to take steps towards more interoperable health data. This collaboration will launch a three-month pilot project focused on improving safety, health outcomes, and quality of life for residents in long-term care. The pilot project will test the use of advanced bed sensors developed by Esprit-ai, integrated with an AI-powered alert system. These sensors, which do not use cameras, provide non-invasive, real-time monitoring of residents’ sleep patterns. This technology enables care staff to more effectively track sleep quality, identify when residents need repositioning to prevent pressure ulcers, quickly respond to falls, and attend to restless residents—all while maintaining privacy and without disturbing residents who are sleeping soundly. By tracking sleep quality, the system will also alert day-shift staff to residents who may be at an increased risk of falling due to poor sleep the previous night, empowering caregivers to take proactive steps in preventing accidents. "Falls remain the leading cause of injury-related death among Canadian seniors, and hospital stays following falls often keep residents out of their familiar environment for extended periods," said Dr. Pooja Mishra, Executive Director of Care at Menno Place. "This partnership with Esprit-ai is an important part of our ongoing efforts to reduce falls, improve resident safety, and support residents in aging in place with dignity." Esprit-ai’s platform offers real-time data to caregivers, enabling them to make informed decisions that improve both the safety and care of residents. Complement or complicate? #AI is transforming many aspects of healthcare; revenue cycle management, operations, diagnostics, treatment management, & patient monitoring/communication. As AI revolutionizes care delivery, it’s also changing what it means to be a physician. Are we training future physicians to meet this challenge? A recent study from Ilker Hacihaliloglu et al. (https://lnkd.in/gSgrXvQv) outlines a solid framework for AI education in Canadian medical schools, offering lessons others can learn from: 🟢 Need for AI Literacy: most medical students & practicing physicians lack training to effectively and ethically integrate AI tools into education & clinical care, hindering appropriate AI use in medicine (https://lnkd.in/gZmngTY3). Physicians need to understand the capabilities, limitations, & ethical implications of AI in healthcare. 🟢 Ethics: From data privacy to algorithmic bias to legal implications, AI brings unique, & not easily approached, ethical challenges. AI education can be embedded into existing courses—such as biostatistics, ethics, and clinical rotations—to give students hands-on exposure while maintaining a focus on traditional medical training. But that requires AI literate/knowledgeable senior educators/mentors (currently lacking).
