The FDA is working to promote AI best practices globally through the International Medical Device Regulators Forum and accommodate AI in clinical trials through the International Council for Harmonisation. The published perspective highlights the need for flexible mechanisms, transparency and proficiency in regulating AI. It also emphasises life cycle management and post-market performance monitoring, especially for large language models (LLMs) and their uses. Dr. Zeina Morcos, co-author of the study, presented her findings at the #CHEST 2024 annual meeting in Boston. The study compared the abilities of two AI models, ChatGPT -3.5 and DoximityGPT, in diagnosing PE. It found that both models could successfully reach conclusions that aligned with clinicians’ assessments and diagnoses of PE made using CT angiography (CTA). DoximityGPT, in particular, showed high diagnostic accuracy, matching clinical diagnoses in 7 out of 10 cases. As a healthcare professional, the recent findings from "Australia's AI Imperative" report by John Mangan are impossible to ignore. Australia's narrowing economic base, as evidenced by its declining Economic Complexity Index (ECI), leaves us vulnerable to economic shocks. By integrating AI, we can: ✅ Boost GDP by $200 billion annually ✅ Create 150,000 jobs by 2030 ✅ Diversify our economy ✅ Drive sustainable growth The implications for our healthcare system and the nation's economic growth are profound. 🔑 Key Findings: ▪️ Without widespread AI adoption, Australia risks losing A$35.7 billion per year until 2027. ▪️ Embracing AI could boost GDP by $200 billion annually and create 150,000 jobs by 2030. ▪️ AI is projected to add US$15.7 trillion to the global economy by 2030. ▪️ Early adoption could secure a 26% GDP increase for Australia. ▪️ The data show an increase in GDP from 2023-2027 due to expanded AI of between 22.5% (US $395 billion) and 28% (US $486 billion), implying an average growth rate for the AI-infused economy of 4.43% to 4.82%. The Australian Digital Health Agency has launched an ambitious five-year roadmap, developed in partnership with the Department of Health and Aged Care and Services Australia, to significantly increase the adoption of national healthcare identifiers. This strategic plan, spanning from 2023 to 2028, aims to revolutionise health information exchanges throughout Australia by ensuring that healthcare identifiers are universally employed across all digital health projects and platforms. The roadmap delineates a series of targeted actions including legislative reforms, enhancements to services, and technical upgrades, all designed to facilitate seamless integration of healthcare identifiers. These unique numbers are essential for accurately identifying individuals, healthcare professionals, and organisations, thus minimising errors in patient data and eliminating unnecessary repetition of patient histories across the health system. Simon Cleverley, Assistant Secretary of Digital Health at the Department of Health and Aged Care, underscores the benefits: “With wider adoption of healthcare identifiers, Australians will no longer need to repeat their medical histories as they engage with different parts of the health system. Newly released, the Practice Cloud Financial and Operations Module, supports General Practices and Aboriginal Member Services to optimise revenue, save time and enhance practice performance to deliver both financial sustainability and improved healthcare outcomes for their practice population. What role did the VC ecosystem play in the MediSecure breach? Last year, MediSecure lost a government contract to manage national electronic prescription services in Australia, a critical hit to their financial stability. This challenge likely contributed to their subsequent data breach, exposing vulnerabilities in their data security measures. But this isn’t just a MediSecure problem — it’s a systemic problem. The principle of data minimization, or "you can’t lose what you haven’t kept", has never been more relevant. Yet, a significant portion of data breaches can be traced back to third-party service providers, with the rigour in assessing these providers varying significantly. Regular audits during the contract period are uncommon unless serious issues arise. This situation with MediSecure illustrates several broader concerns in the venture capital-backed startup ecosystem: • Vulnerability Post-Funding: The loss of the government contract showcased how dependent MediSecure was on external funding to maintain operational standards, including crucial cybersecurity measures. This dependency is emblematic of a broader trend within the VC-backed startup ecosystem, where continuous capital influx is often necessary to sustain operations. • Short-term Focus and Risk Management: Venture capital can push startups towards growth trajectories that might neglect the building of robust internal structures needed for long-term sustainability. In the case of MediSecure, the emphasis might have been more on scaling operations rather than establishing a resilient framework capable of withstanding financial instabilities. • Operational Resilience: The breach suggests a need for startups, especially those handling sensitive data, to develop resilience plans that are not overly dependent on continuous external funding. This involves investing in strong internal risk management processes that can operate independently of financial highs and lows driven by external funding cycles. • Ethical and Strategic Management: For VC-backed companies, there is a critical balance to strike between achieving rapid growth and ensuring ethical management. This includes safeguarding customer data and maintaining trust, which are crucial for long-term success but can be overlooked in the rush for quick expansions and maximising gains at any cost. The MediSecure incident is a stark reminder of the pitfalls of the venture capital model, particularly for companies in sensitive sectors like healthcare data management. To suggest that the upcoming OPC Privacy Act reforms will significantly enhance data security is frankly nonsense. Without a fundamental overhaul in the assessment and monitoring of third-party vendors and a rigorous enforcement of directors' responsibilities, these reforms are unlikely to make any meaningful improvements to our data security landscape. Such systemic changes are essential to address the deep-rooted issues that this incident has brought to light.

Yesterday, I had the privilege of sitting down with Sam Uffindell, Chair of the Health Select Committee, to discuss the state of digital health in New Zealand and the significant impact of the recent funding cuts. Our conversation was a critical step in highlighting how these cuts have stalled essential digital health initiatives, affecting the whole ecosystem. The setbacks not only impact current care delivery but hinder progress toward a connected, modern health system. At the Digital Health Association (DHA), our mission is to represent and amplify the voices within the digital health ecosystem. We are deeply concerned that without strategic investment, New Zealand will continue to fall behind other nations in digital health innovation and adoption. Digital health is not just an added benefit but a necessity—it supports efficient care delivery, boosts productivity, and leads to better patient outcomes. This message is one we will continue to advocate for with both industry and government stakeholders. One of the longstanding challenges in New Zealand’s healthcare system is the “postcode lottery,” where the quality and accessibility of health services depend heavily on geographical location. While this has been an ongoing issue, the recent funding cuts threaten to exacerbate it. Digital health is a critical enabler for connecting care and overcoming geographical barriers, ensuring that all New Zealanders, regardless of location, can access consistent and high-quality care. Without investment in digital solutions, the ability to bridge these gaps becomes increasingly difficult, reinforcing inequities that already exist. Sam and I also touched on the importance of building a resilient health system that can adapt to current and future challenges. Strategic investment in digital health is essential for creating an infrastructure capable of withstanding crises and evolving healthcare demands. The trade offs and opportunity costs of not funding digital in the right way are vast and hurt an already struggling system. DHA remains unwavering in our advocacy and representation of the sector and our health system. We continue to call for renewed focus on the transformative role digital health can play in shaping an equitable, connected, and efficient health system for New Zealand. Our meeting was a strong reminder of the collaborative efforts needed to ensure that digital health becomes a cornerstone of our healthcare framework, fostering sustainable growth and positive health outcomes. I love the way that Clair Sullivan thinks (with and influenced by so many other incredible people!). At #HIC2024, talking us through findings from research looking at the impact of digital maturity on health system outcomes and broader impact. An example - implementation of an electronic medical record in QLD Healrh hospital significantly reduced medication errors and associated harm. Demonstrated that the higher the digital maturity of a health service/system, the lower the staff turnover, the better staff feel they can keep the record updated, understand the patient journey and deliver high quality care, with better patient experience. But it’s hard to justify large costs if thinking stays limited to financial models, using short-term business cases looking to recoup the cost of IT purchases. Longer term mindsets that consider spending on digital technologies in similar ways to how we consider high schools, for example, as an investment, as assets that return value over time. This work is shifting mindsets beyond the delivery of benefits to the creation of impact for societies.

Aged care providers face many challenges. They operate in a heavily regulated industry, with stringent compliance requirements and regulations to adhere to. The funding they receive is finite, but their costs are ongoing, putting a constant squeeze on their margins. For those with an interest in what is going on in #HealthNZ, it has been a tumultuous time. I recently penned my thoughts down and this has been published in the latest edition of Health Informatics New Zealand (#HiNZ) CONNECT magazine. Always the optimist, I strongly believe that Health NZ has the opportunity that Australia does not, and that is to make a handful of national strategic platform decisions, and lay the foundations for an enviable interconnected and interoperable health system. This vision is shared by many, however, something needs to happen to get the ball rolling. We currently seem stuck in the paradigm that 'platforms cost money, so we can't afford them'.
