Primary Care should be Funded as a Common Good The recent publication in the NEJM by Dr Mandy Cohen, Director of the CDC, describes the underused and much needed synergistic power of integrating Primary healthcare and Public Health. This synergy is underpinned by the reality that Primary Care and Public Health are the “Twins Separated at Birth” with both having the common DNA that allows them to be recognized as “Common Goods”, that both benefit society as a whole.  Public Health has long been accepted and funded as a Common Good but Primary Care recognition has lagged behind until recently.  The CDC Director makes the important point that both Public Health and Primary Care need to be sufficiently funded if they are to serve the “Common Good” adequately.  Public Health has been underfunded by states and the federal government. Primary Care has been underfunded by insurers (Commercial and Federal). The clear path to increasing Public Health funding is through government budgeting. The path to increasing Primary Care funding is more complicated and must pass through the hands of many insurers and intermediaries with diverse priorities (including profit and power) that may or may not be directed at using Primary Care to bring better health to people and communities. The 2021 NASEM report “Implementing High Quality Primary Care” suggests that Primary Care is a common good and should be provided with government funding in the same manner as public health. The report offers a governmental policy framework to support, fund, define required elements and hold primary care accountable. As we navigate this framework there are a number of important issues to address, including but not limited to: What does funding primary care as a common good mean? How should the “common good” be funded; capitation, or an enhancement to current funding models? Is moving forward to fund PC as a public good a state-by-state decision process, federal decision or hybrid? Must states and/or the federal government establish non-exhausting funding mechanisms? Must funding be linked to new or existing tax revenues and how will sufficiency of current and future funding be established? Will the current insurance providers remain as intermediaries and how will the Public Good funds be paid? How will primary care accountability be established and enforced? How will PH/PC integration be assured? 👍🏽 Congrats to my colleague Khaled Aldahmani, and his co-authors Mohamed Suliman, Khadija Hafidh, Salem Beshyah on their recent publication in the Journal of Diabetes and Endocrine Practice. https://lnkd.in/dgFH8Guw 📔 The researchers conducted an extensive review of the current literature regarding the epidemiology, complications, and quality of care, as well the challenges of diabetes management and future directions in clinical practice and research. 🇦🇪 Key findings: 👉🏽 The International Diabetes Federations estimates the prevalence of diabetes in the UAE at 12.3% for the 20 to 79 age group (age-adjusted 16.4%). Other prevalence studies reported diabetes rates between 19 - 25%, depending on the population and age studied. 👉🏽 Many research studies in the UAE found a relatively high prevalence of undiagnosed diabetes before the survey. This number (25-40%) has remained remarkably consistent over many, many years. For example, one study amongst Emirati students (17-25 years) found a prevalence of 24% prediabetes and 9% diabetic.  👉🏽 The UAE has been an early adopter of CGM technologies. Glucose monitoring devices are available to most patients with diabetes in both public and private health care facilities. Several groups have published their CGM experience in different patient populations and clinical settings. 👉🏽 UAE research productivity related to diabetes has increased dramatically: from less than 50 research publications in 2013 to over 300 publications in 2023.

What’s the HEC? Following a long period of advocacy and supported by a trust fund of $1 billion, the Agency for Healthcare Quality and Research (AHRQ) has signaled they will soon release a Notice of Funding  Opportunities (NOFO) for state-based Health Extension Cooperatives. The intention of this NOFO is to demonstrate the effectiveness of state-based healthcare practice transformation support systems. This is similar to the way in which the Dept. of Agriculture has shown decades of success in the Regional Extension Cooperatives (RECs) to support rural farmers. This program comes at a very opportune time as 20+ states are actively engaged in transforming the payments and delivery to primary care. CMMI is promoting state-based payment and primary care delivery reform in their Making Care Primary Program, the AHEAD state-based model, and the recent ACO Flex model. All of these programs require providers to undergo major changes in the way they deliver care, expanding their focus to population health while delivering patient centered comprehensive care. The programs also compel states to look carefully at how the health of their diverse, rural and underserved communities can be maintained and improved over time. Change does not occur spontaneously and none of the hopeful changes in these programs are easy to accomplish. The introduction of HEC resources, regionally distributed within a state, is a powerful way to meet the educational and resource needs of the states and providers as they begin the journey of transformation to high value, whole person equity focused care.  There are a few good examples of the effectiveness of "HEC-like" programs in Vermont’s Blueprint for Health and New Mexico’s Health Extension Regional Organizations. My experience in the Maryland Primary Care Program also demonstrated the essential role of the State Health Department's Program Management Office and regional Care Transformation Organizations providing technical assistance and resources to primary care practices to successfully integrate behavioral health, care management and advanced health information technology in primary care practices across the state. In a historic move toward global public health enhancement, the 4th High-Level Global Ministerial Conference concluded its sessions in Jeddah, Saudi Arabia. The conference set forth a comprehensive new roadmap to address the challenges of antimicrobial resistance (AMR). It underscored the commitment of member states to the strict and determined implementation of proposed strategies within an international cooperative framework to curb the spread of antibiotic resistance and ensure the efficacy of these crucial treatments. The conference's activities included in-depth discussions and interactive sessions on the latest research and discoveries in this field, alongside showcasing successful national experiences in curbing the spread of antibiotic-resistant infections. Emphasis was placed on the importance of raising awareness among healthcare workers and the general public about the optimal use of antibiotics through ongoing awareness and education campaigns. #ActOnAMR #4thMinisterialAM
