So here is good news updates for my GP doctors followers in UAE.Some of my GP doctor friends suggested to me that a visual educational YouTube videos on various topics would attract more views from my followers as I am actually a good teacher with strong communication skills.I felt videos I shoot myself are of poor quality and such videos even short duration ones must be done by professionals where visual and sound quality can be ensured.So I am in the process of doing something similar simple yet effective for my GP doctors and fellow junior internal medicine specialists doctors.I shall be focusing on core internal medicine topics like asthma fevers hypertension dyslipidemia and Ofcourse latest therapies in diabetes.The purpose of these videos shall be on how to approach an internal medicine clinical case when faced with one and how to decide which treatment modality shall best suit the patient.Also there will be a small announcement on July 11(details awaited) but main news is my plans to set up a professional YouTube channel to benefit junior doctors in uae #information education

Today’s case discussion is on 2 things.One is lot many GP in A clinics at Dubai diagnose Urti wrongly as viral without examining throat and very many patients come to internal medicine specialists with high grade fever and sore throat which was not visualised by GP doctor.This is a wrong approach as many bacterial infections are being missed and patient suffers.You can label as viral only if wbc count is below 3000 or if influenza is positive.Else there must be some lab or clinical evidence for suspecting as viral.Proper physical examination is mandatory too. Second point is the wonder molecule tirzepatide which many patients are buying even in the black market to lose weight.While it may or may not be superior to other Glp1 (I was one of the main doctors who popularised Glp1 in Kerala way back in 2016) but same company made me a speaker only in 2018.Now I observe in Dubai their team focuses only on certain specialities and ignores the internist to whom many patients approach for tirzepatide for weight loss.As insurance here approves it only for diabetes I am not over prescribing it but my experience is better with the oral version of Glp1 as the promoters of tirzepatide are not engaging with internists enough at Dubai #Glp1 agonists

According to the WHO the number of Saudis older than the age of 50 years of age is expected to be 18 million in 2050 comapred to 6 million in 2023. This is around 3 folds increase and while this age group constitutes 16% of the population today, it is going to constitute 37% of the total population then. Coupled with that we should expect tripling in all age related diseases, and dementia and Alzheimer’s disease are on top of them. We should start now to develop the needed infrastructure and train needed specialists to face this expected challenge and more importantly we should emphasize the importance of healthy brain behaviors as according to the Lancet report in 2020, 40% of all cause dementia is preventable. #alzheimer’s #brain_health_initiative #dementia #Saudi_Arabia

Issuing this post in interest of GP doctors at Dubai. There is a trend in Dubai by locals and expats to take blockbuster drugs like mounjaro and ozempic over the counter from pharmacists or GP doctors without or with prescription. This is wrong. Neither drugs is approved for weight loss in UAE. If in doubt refer such patients to local doctors from ministry internal medicine specialists or endocrinologists. Don't encourage them to take Glp 1 without certain criteria like hba1c above 7.5 fasting sugars more than 120 and bmi above 30.There are cases of patients dying due to paralytic ileus by taking glp1 without prescription or meeting criteria. It's approved for use in USA and UK for obesity alone and only in those countries. UAE is yet to frame guidelines for use in obesity alone so doctors have to respect UAE rules. Also though novo nordisk is meeting non endocrinology doctors Eli Lilly hardly meets any internists or GP doctors so there is no need to promote these molecules for obesity. However as diabetics medications they are good for post prandial sugar control and may benefit obese diabetes cases in UAE. #diabetes
