Consider the role of complexity not from the top down or the designer perspective, but from the bottom up - the patient and delivery team member perspective. You might see the increasing challenges of More to do (micromanagement, digitalization, regulation, certification) fewer team members (lower ratios) and lesser delivery team members (replacements of experienced team members by less experienced, made much worse by massive expansions of annual graduates as in NP PA and DO) ... to share the increasing complexity in many dimensions (patient, practice, community, relationships, environments, insurance quality poor made worse) ... and despite less time ... and despite disruptions of personal and professional lives ... that prevent best on on one innovation with each patient ... thus shaping moral injury, burnout, turnover, higher turnover costs, and even worse finances in practices and hospitals

"However, these savings—as much as $50 billion annually1—are unreported and not shown to improve care access for patients without adequate insurance.2,3" ---Well, the financial design is so bad that it has closed over 600 hospitals since DRG and the era of cost cutting. The financial design is the reason for closures and compromises. The 50 billion distributed to hospitals serving vulnerable populations (40 - 50% of US pop) would be some help due to the Era of Cost Cutting 1983 to 2030 or longer. "Per CMS’s budget neutrality principle in the Outpatient Prospective Payment System (OPPS), the $9 billion repayment must be offset by $7.8 billion of cuts to nondrug reimbursements." ---Budget neutrality and regulatory capture prevent any true reform to add support to the hospitals and practices serving most Americans most behind. Once again CMS is not accountable and must be reigned in even if it takes 50 years as it did for physician accountability over the last half of the 19th century and for human subject researchers over the last half of the 20th century. Medicare and Medicaid have failed for 2621 counties (40% of US) most behind and has primary care and basic health access except 1965 to 1978. What about the Belmont Principles and numerous violations? What about protection of vulnerable populations, informed consent (which requires prior study of consequences poorly done by CMS), or beneficent intent - especially with all three lining up consistently against the majority of the nation most behind time after time! Strike 1 for ethical and moral violations - CMS has demonstrated time and again the inability to protect their charges - vulnerable Americans. CMS most fails where there are concentrations of elderly, poor, disabled, and their worst public plans. Strike 2 - CMS has failed to do adequate studies of consequences prior to the implementations of policies since 1983 and including Medicaid Waivers which will steal more from team member support and give more for team members to do - with worst impacts on Community Health Centers with 50% Medicaid and in the most vulnerable populations highest in concentrations of Medicare, Medicaid, and Dual plans. Studies are done more to justify what CMS is doing rather than to protect most Americans most behind from consequences such as their providers rated poorly or penalized just for caring for these populations inherently lower in outcomes and drivers of outcomes (Readmissions, ACO shared savings, Star Ratings, others) Strike 3 - CMS fails for beneficent intent. The intent is cost cutting. CMS harms tens of millions with designs that have killed hundreds of hospitals and countless practices, delivery team members, and local leaders in health and other community areas. We know about some economic and access impacts from these losses, but the full impact and cascades of impacts are never studied. Fascinating new research from Dr. Jeralyn Bernier Jacobs, a physician research scholar at Mid-Atlantic Permanente Medical Group | Kaiser Permanente, shows a significant rise in the incidence of #prediabetes and #TypeIIDiabetes in children and adolescents early in the COVID-19 pandemic. This study explores the outcomes of pediatric patients with status asthmaticus (SA) who received either inhaled volatile anesthetics (IVA), extracorporeal membrane oxygenation (ECMO), or both as rescue therapies in a PICU setting. Not only are midwives being challenged by health care systems to practice to the full scope of their education and licensure, but even growing a workforce of midwives is now significantly diminished by UCSF School of Nursing mandate to move the historic masters graduate midwifery program to a DNP. Why? This article hit the San Francisco Chronicle today! I hope the readership and citizens of CA can understand significant barriers exist to not only practice midwifery, but now to become a midwife, particularly those from underrepresented groups. Once again we see the ramification of profit over purpose. There are NO published studies that demonstrate value for cost as an entry degree to practice in midwifery, or any other advanced practice provider role. I urge heightened pressure for this decision to be reversed and reinstate a historic and predominantly BIPOC faculty remain as educators. The dismantling of the midwfiery program is beyond reason and provides suspect to many inherent struggles within the SON. #Enough #GrowMidwives

The inferiority of the American Illness Profit System becomes clearer day-by-day, here is just the latest in an endless sequence of reports showing a factual basis proving its inferiority. Yet nothing happens in Congress to correct any of this because the Congress members are so corrupt, and in the pocket of the corporations that control health care in this country. The only thing that is going to change this is how you vote in what may be the last real election in which you will ever be able to vote.

Staffing Guidelines. In a recent article published in AACN Advanced Critical Care, the collaborative efforts of L.A. County Department of Health Services (DHS) hospitals were highlighted as they worked together to develop unit-specific guidelines for staffing critically ill patients. A 2021 report by the The National Academies of Sciences, Engineering, and Medicine found that while more than 35% of health care visits in the U.S. are to primary care physicians, only about 5% of health spending is on primary care. “Vaccine requirements in places like schools or hospitals are important for preventing the spread of disease and protecting vulnerable people and kids,” - from my interview with Colorado Public Radio School vaccine requirements play a crucial role in boosting vaccination rates, creating healthier schools, protecting children, and strengthening community health. Most parents agree: a nationwide survey this year found that 77% support vaccine requirements for school attendance, while only 8% believe they are unimportant (source in comments). At the Rocky Mountain Poison Center, we are driven by our mission to deliver healthcare that helps individuals and communities thrive. One of our goals is to prevent poisoning injuries and create safer homes for families. Inspired by the concept of "nudges," as described by Richard Thaler and Cass Sunstein in their book “Nudge: Improving Decisions About Health, Wealth, and Happiness”, we have implemented a series of strategic interventions (nudges) to guide people toward safer behaviors without limiting their choices. One such intervention involves nudging callers after their health encounter with us to safely dispose of unused prescription medications, helping reduce the risk of exploratory ingestions by children. During a call, it can be challenging for callers to fully focus if they are given too much information at once, especially when they are worried about a potential health threat. After a call, we can send a follow-up text message with the location of the nearest safe drug disposal site, based on the caller’s location. This follow-up text serves as a helpful reminder or nudge, allowing callers to take action once the immediate health concern has passed. This simple nudge makes it easier for families to remove potential dangers from their homes. We are encouraged by the great work of M. Kit Delgado and Penn Medicine and the coming work by Jason Mansoori from Denver Health in utilizing nudges.
