1 For Further information on completing this form see SMS 05-02-00
2 This colour shows a field for data entry in the form Calculated / linked field.
3 Date shall be in this format- Date (dd/mm/yyyy) - TIME shall be in format hh:mm
4 All specific data must be filled in the 1st sheet (start up) before using any forms in MAR002.
5 Please use the appropriate Operations / Tank type where applicable and the relevant data. Mark N/A if required.
6 Some of the information may not be specific to the tank type and shall be left blank or mark N/A.
7 During the Port Stay, all applicable hourly record must be made and compared.
8 A complete record during the voyage must be made into the respective form.
9 Where applicable, snapshots / log printouts shall be taken (soft or hard copy).
10 All Cargo / Ballast quantity shall be entered in the m3.
11 Mention the unit of the pressure as applicable.
12 Use Tank Cooldown at Sea Form for extended cooldown in Port also.
Users are encouraged to use the Electronic copy as much as possible. Where applicable, print layout is set for 24

hours of the monitoring sheet only.
14 Subsequent Hourly Time will be auto filled once the minimum data is entered.
15 Frequency / report / retention – refer to SMS 03-03-03.
