Introduction
Surveillance officer app is very vital in the SORMAS platform which serve as center spot for reporting
disease outbreaks from a designated Local Government Area (LGA) of operation, by the Local
Government Area Disease Surveillance and Notification Officer (LGA DSNO)

User interphase work flow


Step 1

OPEN the SORMAS app on the tablet desktop by a single tap on the
SORMAS icon

Step 2

Enter the SORMAS Authentication PIN


5050 hit the enter key

Step 3: The surveillance officer user interphase opens with the following

Name of Login LGA and picture of Surveillance officer

TASK: The menu view which shows created and received tasks

CASES: Shows details of all disease cases entered on the app and also
new cases can be created via this menu

CONTACTS: Shows details of Contacts from a source case of VHFs e.g.


(Lassa fever) which require contact investigation

EVENTS: Shows details of Events information’s it could be case of a


Rumor which could also be created

SAMPLES: Shows details of Samples entered into the platform under


investigation and resuits

REPORTS: Shows summary of all reported cases on the device it could be


to submit weekly report by the surveillance officer

NB: Breakdown of details of menu will be given below

Tasks Menu
The menu task is a very vital, it role is simply to assign to a subordinate
(Surveillance officer, Contact officer, or Hospital Informant) and receive from a
supervisor (Admin, National user or State supervisor).

The tasks could be of different types

 Other task as described in comments


 Generate daily report
 Generate surveillance report
 Generate weekly report
The priority of the task should be indicated (High, Normal, Low) with start date
and end date of the task should be correctly indicated

The Drawer icon which reveal more details about tasks weather it is (Pending,

Cases Menu
This is the most useful menu in the surveillance app menu it shows all the
cases listed on the app (Case Listing)

NB: All dialogue boxes should be filled correctly as applicable to you

New case Icon: Use to add a new case

Menu icon: Use to navigate the surveillance app

Drawer icon: Use to Navigate within case properties

The drawer icon opens up to reveal filter (Pending, Done and Discard Cases)

The case search for a specific which appears below are generic such as

 ID, name or EPID Number: NIE-KBS-BES-19-01, Samuel John


 Case origin: In-country or Point of Entry
 Disease: Lassa, Cholera, Measles, Yellow fever, Ebola etc.
 Case classification: Suspected, Confirmed, Probable case Not a case etc.
 Outcome of case: Deceased, Recovered, No Outcome yet, unknown
 New cases from Epid week to via: Wk 35-2018(27/08-02/09)

New case Entry (All variables needed for case should be added correctly before saving to avoid duplicate
(double entry)

Save Icon: Use to save entered details

NB: You will encounter this page on several occasions. This is as a result of a case
listed in the system with the same set of variables such as (Name, last name).
The system required you to do either of the following:

Update Search: To continue with the existing case

Create: To create a new case with the variables listed (name, last name)

Cancel: to exist the page

NB: For all cases entered into the there are two modes
Read Case: To view all the variable entered into the system but cannot be
edited for a particular case
Edit Icon: Tab to enter the Edit Case (Pen Structure)

Edit Case: To review all the variable entered into the system for correction
for a particular case
Save icon: use to update the variables on the platform

NB: All variables are expected to be entered correctly to avoid mistakes

Drawer Icon: Have eight (8) sub-listing which is required to be filled by the
surveillance officers as listed below.

1. Person information: Case demographic such as Name, Age,


Gender, Education etc.
2. Case information: Details such as case classification, Outcome,
Epid number etc.
3. Hospitalization info: Facility which the patient was hospitalized
4. Symptom information: case symptoms with date of symptom
onset
5. Epidemiological info: Epid-link of case with possible source of
infection
6. Case contacts: Contacts listing with source case in case of VHFs
(Lassa, Ebola)
7. Case samples: Samples of case with testing Laboratory with type
of test
8. Case tasks: Request for specific task for case by the Surveillance
officer

Enter patient location and pick current
GPS for Address of case and select OK

Epid Number is auto generated once a new case
is created.
Case Classification: Click to Select, it depends on

New contact Icon: Use to add contacts for a source case

This is a major menu use for contact Tracing for VHFs. Once a contact is
identified and listed, by the surveillance officer it is the duty of the
contact officer to follow up on such contact which will be broken down
step by step as we move on.

Enter the new contact info:


 First name, last Name
 Date of last contact with source
case
 Type of contact, Relationship
with source case
 Contact description
Click save icon to save details

Create a New sample


 Enter Date of sample collection
 Select type of sample to be use
 Select Laboratory

Contact Menu
The Contact menu on the surveillance app show the list of contacts
that are under follow-up and their status

Confirmed case: A suspected case with a laboratory confirmation of a


particular disease e.g (Lassa fever)

Source case: is also known as a Confirmed case with listed contacts

There are three classification of contact


Confirmed contact: A suspected case with a direct or indirect contact
(Epi-link) with a source case or Corfirmed case
Unconfirmed contact: A suspected case with suspision of direct or
indirect contact (Epi-link) with a source case or Corfirmed case
Not a contact: A suspected case with no direct or indirect contact (Epi-
link) with a source case or Corfirmed case
Contact under follow-up: Are Confirmed contact who are under
investigation during the incubation period of a particular Viral
Heamoregic Fever (VHFs) e.g (Lassa fever and Ebola)
Contact Listing: are list of contacts form different source case for listed
VHFs

From the drawer contact are sorted into:


Ongoing: Contact under follow-up
Completed: Contact who have completed
follow-up
Canceled: Contact whose follow-up was stoped
Lost to follow-up: Contact who decline follow-up
No follow-up: Contact not under follow-up

For each contact listed there are set of information which needs to be
compeleted

Contact information
Person information
Follow-up information
Contact task (same procedure for task)

The source of a cases can be opend


Clicking on (Open source case)

If any contact become smptomatic and laboatory confirmed positive during


follow-up such contact can be converted to an independed case this can be
easily by clicking on (Create case for this contact person)
Events Menu
The event menu is used to report details of suden public health suitation or
rumor from a particular community
New event Icon:use to add new event of rumor
Enter event details
 Date of event
 Event status
 Event description
 Disease
Click on save icon after all entry

The Sample menu shows the list of all samples listed from cases for diagnosis

Not shipped: Samples not sent from app
Shipped: Samples sent from app
Recived: Samples recived at laboratory
Referred: Samples posted to other laboratory

Click on listed samples to view details


Click on Edit Icon to edit sample

Click on save Icon to save sample

Report Menu
 The report menu show Aggregte details of reported cases,it enable the
surveillance officer to send weekly report of Zero report
