Definitions

Term Definition

Contractor The contractor (and subcontractors) is the party which carries out all or
part of the design, engineering, procurement, construction, commissioning
or management of a project.

Energised system Energised system means connected to a source of electrical supply and/
or subject to hazardous induced capacity voltage

Isolation Disconnected or separated from all possible sources of energy (electrical


or mechanical).

LOTO Lock Out Tag Out (LOTO) is a safety procedure implemented to ensure
energised systems are locked off and de-energised before work is
conducted on them.

Supervision The Supervision Consultant is the party that provides engineering-related


Consultant services such as design, supervision, execution, repair, operation,
maintenance, technology, creation of drawings and specifications, and
makes recommendations to their employer.

1 Scope of Procedure

This document highlights the Lock Out Tag Out (LOTO) procedure for xxx (electrical/ mechanical)
activity to prevent danger from electrical and mechanical hazards during works on energised system.

A robust lock out tag out procedure reduces margin for error and ensures clear roles and
responsibilities between workers of different organisations.

The procedure is split in 3 processes

 Preparation
 Isolation
 Restoration

The xxx activity will be carried by “contractor name” out at xxx location.

Other Parties involved for coordination includes:

1 Control of Work
1.1 Permit to Work Procedures
Permit-to-work systems and LOTO procedure will be implemented for works on energised systems as
these high-risk activities to ensure compliance with Dubai Municipality and Expo requirements.
Liaison with all relevant parties, will be ensured where required. Permit to Work forms are included in
Appendix A.

Xxx contractor PTW requirements will be adhered to.

3 LOTO Methodology/ Procedure

The process complies to the Expo Assurance Standards as shown in the below figure:

1.2 STEP 1 - PREPARATION


 Development and approval of task specific MSRA.
 Isolation permit is requested.
 Approved MSRA and permit requirements will be communicated to the trained operatives,
records and copies of documents will be made available on site.

1.3 STEP 2 - ISOLATE

The following steps will be completed:


 Identification of the energised system to be isolated
 Communication of isolation to affected parties.

1.4 STEP 3 – SHUTDOWN

 Identified energised system is shut down and secured.


 Minimum of 2 locks and 2 tags is applied. (where more than one party is involved, each
electrical duty holder must posses at least one lock). Tags must clearly identify key holders
name and reason for lock out of energised system.
 Isolation is verified to ensure de-energisation/ no electrical energy is present on the isolated
system using a calibrated meter.
 Isolation permit is accepted and handed over to the electrical duty holder.

1.5 STEP 4 – ACTIVITY COMMENCES

 Only the competent electrical duty holder will be responsible to carry out the works on
energised system. Certificate will be available on site, task specific personal protective
equipment is provided and used i.e Anti static gloves.
 List works to be done on isolated system. Eg. Cable connection, maintenance, termination
etc.

1.6 STEP 5 - ENERGY RESTORATION

 After completion of works, area is cleared off any debris, unwanted materials etc.
 Works checked for completion.
 Tags are removed by the individual electrical duty holders.
 Energy is restored.
 System operation is checked and confirmed working in normal condition.

2 Project Organization
2.1 Roles & Responsibilities

3 Access & Lighting Arrangement

Please state the proposed access method of how operatives will access and work in the area. This
may include use of scaffolding systems, mobile scaffolds, podium steps, MEWPs, cradles etc. Work
areas shall have 2 means of access and egress where possible and shall follow the work at height
hierarchy of control. Standards for the protection of any pits or floor plate removal protection should
also be included here to prevent falls below ground level.

This section should also state whether access will be temporarily blocked or rerouted due to the
activity.
Specify lighting arrangement in planned work area.

4 Tools & Equipment


The following plant and equipment are required for the works

 Antistatic electrical gloves BS EN 420 compliant


 List all other tools, equipment and plant that may be required to implement this procedure e.g.
MEWP, scaffolds, platform ladders etc.

4.1 Personnel Training Certification

 Electrical Duty Holder – Certificate must be attached.
