3. TERMINOLOGY & ABBREVIATIONS


On Site Doctor : Profession who have the authority to perform medical practice and health
care program which is working in the company and run health services for
workers in [Link]
Paramedics : profession that provides pre-hospital and emergency medical services which
is working in the company and assist doctor running health services in
[Link]
ECG : Electrocardiogram is medical tests to detect heart abnormalities by
measuring the electrical activity which is produced by the heart

4 RESPONSIBILITY
4.1 On Site Doctor
1. Perform ECG Test to employees
2. Determine FIT or UNFIT health status of employee by assessing ECG result
3. Make recommendation letter to HRD if needed
4. Make referral to specialist doctor if needed
5. Make referral to laboratorium for additional/special test if needed

4.2 Paramedics
1. Assist doctor in performing ECG Test to employees
2. Perform ECG Test if doctor not available or doing other job.

4.3 Occupational Health and Safety Manager


Ensure ECG Test running well

5. TOOLS AND EQUIPMENT


Tools and equipment required for ECG Test are :
1. Bionet “Cardio Care 2000” or other ECG Tool
2. ECG Gel “Cardio Gel” or other same type gel
3. ECG Paper
4. Tissue

6. PROCEDURE
1. Patients are instructed to open clothes, socks, watches or other metal tools
2. The patient is instructed to lay down on his back on the bed provided
3. In areas / locations that are installed electrodes, first cleaned of dirt / sweat, then apply
the gel sufficiently
4. Install the electrode as follows :
4.1. Installing electrodes of extremity (wrist and ankle)
4.1.1. Right ankle – BLACK cable color
4.1.2. Left ankle – GREEN cable color
4.1.3. Left hand wrist – YELLOW cable color
4.1.4. Right hand wrist – RED cable color

4.2. Installing electrodes of the chest


4.2.1. V1 : Right parasternal intercostal IV
4.2.2. V2 : Left parasternal intercostal IV
4.2.3. V3 : In the middle of V2 and V4
4.2.4. V4 : Left midclavicularis
4.2.5. V5 : Left axillaris anterior
4.2.6. V6 : Left midaxillaris

5. Look at the ECG unit, the heart rate recording will be visible, leave it for a few seconds.
If the "ready" light is on, then press the Start / Stop button
6. Wait until the results and other comments come out.
7. If there is an error on the ECG screen, check the electrode first to see if something is
loose. If you have, repeat the examination and instruct the patient to relax
8. After printing out the ECG tape comes out, release all the electrodes on the patient's
body, clean all the electrodes and rearrange them so they are ready to be reused
