Comprehensive Physical Assessment Report

The client has the following physical assessment findings: 1) Vital signs show elevated blood pressure of 150/90 mmHg, tachycardia with a heart rate of 143 bpm, and tachypnea with a respiratory rate of 24 CPM. 2) Cephalo-caudal assessment finds a round head size, dry skin, pale nails, and bruising on the right forearm likely due to IV insertion. Lung auscultation reveals crackles. 3) The abdomen is distended with dull sounds on auscultation and hyperactive bowel sounds. The left arm shows non-pitting edema likely due to poor circulation. Motor function is reduced on the left side
