Yet this study demonstrates that the BDNF locus is more complex than previously recognized.

The brain is one of the most complex systems in the human body and one of the most actively studied in regenerative medicine. On World Brain Day (July 22), we’re highlighting the growing research into how mesenchymal stem cells (MSCs) are being explored for their potential role in supporting neurological conditions such as Multiple Sclerosis, Alzheimer’s disease, Parkinson’s disease, and traumatic brain injury. Our previous work on digital twins in multiple sclerosis focused on inferring changes in neural conduction delays from multimodal imaging, with the aim of gaining mechanistic insight into the ongoing pathological processes: 🔗 https://lnkd.in/extUzw55 🔗 https://lnkd.in/eXGUy5p6 However, this approach left an important aspect insufficiently addressed: secondary progressive (SP) multiple sclerosis, in which disability progression cannot be fully explained by demyelination and altered conduction alone. In our new study, we therefore shifted the focus from white-matter conduction delays to changes in cortical functioning. Using a different whole-brain model, we show that clinically relevant parameters—particularly those related to brain-network excitability—can be inferred from empirical data. Crucially, these parameters distinguish Relapsing-Remitting from SP phenotypes (outperforming standard clinical measures), and predict clinical status (cross-sectionally) best in patients with SPMS. This suggests that different disease stages may need digital twins tailored to the mechanisms that predominate. For #WorldBrainDay, we’re highlighting a paper that captures something remarkable about how the brain responds to injury. A #Stroke or #TraumaticBrainInjury can damage the pathways that carry movement commands from the brain to the spinal cord. The adult central nervous system has very limited capacity to regenerate these circuits, and there’s currently no treatment that restores them fully. But this review in #FrontiersInNeuralCircuits, by Takahiro Inoue and Masaki Ueno (Niigata University), brings together evidence — mostly from mouse and rat studies — showing that surviving pathways can partially compensate: intact fibers sprout new connections into the areas left disconnected by injury, and these new connections measurably contribute to the modest motor recovery seen after injury. The authors focus on three major descending pathways — the corticospinal, rubrospinal, and reticulospinal tracts — and lay out what’s known about each one’s structure and function, as a foundation for understanding how they might be encouraged to rewire more effectively after damage. On World Brain Day, it’s a reminder of how much is being uncovered about the brain’s capacity for self-repair — and of the early efforts already underway to turn that knowledge into treatments. 🧠 Scientific progress grows through constructive discussion. We are pleased to share our response to the Letter to the Editor by Messina et al. on the role of Critical Illness Polyneuropathy/Myopathy (CIPNM) in patients with Severe Acquired Brain Injury (SABI). 🤝 While agreeing on the importance of early clinical suspicion, we emphasize that a Neuro-ICU–Neurorehabilitation co-management model is key not only to identifying patients at risk, but also to sharing who should be screened and when. Definitive neurophysiological assessment is particularly valuable at admission to neurorehabilitation, when sedation has been withdrawn and acute ICU-related confounders have largely resolved. A shared pathway from acute care to rehabilitation can make neurophysiological evaluation more timely, meaningful, and clinically relevant.
