  • Transitions in care are often where patients and families feel the most overwhelmed, especially when multiple specialists, medications, and complex medical needs are involved. That’s why consistent communication, collaboration, and continuity matter. We’re grateful to receive feedback like this about Anton Furman, DO and the care our team provides through First Docs’ 360 Community Medicine Model. “Managing such a complicated medical situation requires communication, patience, knowledge, and compassion, and Dr. Furman consistently demonstrates all of these qualities.” At First Docs, our goal is not simply to provide episodic care after discharge from the hospital. We work to create true continuity across the healthcare continuum through proactive physician involvement, close coordination with specialists, and collaborative partnerships that help patients and families feel supported every step of the way in their recovery. This is the impact of physician-led, relationship-driven medicine.

First Docs was proud to sponsor a table at St. Mary Medical Center, Langhorne, PA's A Night in the Islands Gala this past weekend. It was great to support our friends at Trinity Health Mid-Atlantic and spend time with SMMC leadership, physicians, community partners, and colleagues who care deeply about advancing patient care in Bucks County, PA. The event supported the development of a new Interventional Radiology Suite, which will be an important investment that will expand access to minimally invasive care for patients in the region. A number of our providers were in attendance, including Danielle Steinmetz, M.D. and several physicians who completed their residency training at St. Mary. Congrats on a beautiful evening. Thanks for allowing us to step out of the day-to-day and spend time with partners and friends outside of the clinical setting!

  • Today we recognize National Doctors Day. Our First Docs physicians deliver care for some of our most vulnerable patient populations across hospitals, skilled nursing facilities, and outpatient primary care. The work is complex and hands on. It demands sharp clinical judgment, consistency, and ownership of outcomes in high-acuity, fast-moving and chaotic environments. Our physicians are on the front lines every day. Showing up, making decisions, and supporting patients and families through critical moments and care transitions. That commitment drives better care and sets the standard for what medicine in these settings should be. We are proud and appreciative of all physicians who choose to do this work. Thank you for the impact you make every day.

  • This week we recognize the Nursing Home Administrators who selflessly lead their SNF teams every day. Administrators carry one of the most complex roles in healthcare. They balance clinical quality, regulatory requirements, staffing, family and patient expectations, and operational excellence. As a medical practice serving patients in skilled nursing facilities across the US, we see firsthand how the success of a building depends on strong administrative leadership. Great administrators create an environment where clinical teams can do their best work. They build cultures centered on accountability, responsiveness, and resident-centered care. They support nurses, therapists, and physicians to ensure our shared patients and families receive the attention they deserve. To the administrators leading centers across the country, thank you for the work you do to support patients, families, and the teams who care for them. Happy Nursing Home Administrator Week! #NHAWeek #PostAcuteCare #SkilledNursing #HealthcareLeadership

  • Next week, First Docs will be meeting with long term care leaders from two great states! We’ll be attending: • The American College of Health Care Administrators (ACHCA) NY Chapter 56th Annual Convention March 8–11 | Resorts World Catskills | Monticello, NY • The Delaware Health Care Facilities Association Annual Meeting March 12 | Modern Maturity Center | Dover, DE Across both states, administrators and skilled nursing leaders are facing the same operational pressures: • Higher resident acuity • Avoidable rehospitalizations • Inconsistent physician presence • Delayed medical decision making • Increasing family expectations SNFs require daily, proactive physician leadership. Passive and reactive medical models are no longer sufficient. Stronger on-site physician management leads to better clinical outcomes, smoother operations, and stronger hospital relationships. If you are attending either event, let us know! We welcome the opportunity to discuss how First Docs supports facilities with consistent on-site Medical Director presence and proactive medical oversight. #FirstDocs #LongTermCare #SkilledNursing #PostAcuteCare #ACHCA #DHCFA

  • Today we recognize Women Physicians Day. Across hospitals, skilled nursing facilities, clinics, and post-acute settings, women physicians are delivering high-quality care while navigating some of the most complex clinical environments in healthcare. At First Docs, we see this impact every day. Women physicians lead with clinical rigor, compassion, and consistency, often serving as the connective tissue for patients transitioning across care settings. Their leadership improves outcomes. Their presence improves trust. Their work improves lives. Today is a moment to acknowledge that contribution and to thank the women physicians who care for patients, lead teams, mentor colleagues, and raise the standard of medicine. We’re proud to work alongside you. #WomensPhysicianDay #WomenInMedicine #PhysicianLeadership #HealthcareLeadership #FirstDocs

  • Today we honor Dr. Martin Luther King Jr. and his enduring call for justice, service, and compassion. In healthcare, that call shows up in how we care for our patients, how we support our teams, and how we work to close gaps in access and outcomes for underserved populations. We remain committed to leading with empathy and purpose in the communities we serve.
