While there isn't much gore, the scene where Araragi first encounters Kiss-shot is very bloody, and the severed stumps of her arms are seen, but not explicitly detailed.

Towards the beginning of the film, Araragi steps outside and sets on fire. Pretty intense
Quite a few scenes could be considered intense.
Araragi's first meeting with Kiss-Shot features the vampire sitting in a large pool of her own blood, with her incessant crying and begging for mercy being contrasted with that of an infant's. This could be distressing to some.
