In the film, he has a butterfly tattoo on his back and a US Army Special Forces tattoo on his right arm. The Army tattoo (sword with two arrows crossed) has the motto "De Oppresso Liber" which means "to free the oppressed". The Army tattoo is self-explanatory given his trade, and the symbolism of the butterfly is meaningful if you've seen the film. Both seem to be Jack's because they speak directly to his character. I don't know if Clooney is tattooed in real life.
She wasn't. She was killed because the gun Edward made for her, exploded in her face, as intended by Edward. Remember he made some last minute modifications just before delivering the gun.
Now, if you meant to say why did Jack kill the woman at the beginning of the movie, Ingrid (Irina Björklund), his reason was likely twofold. One, in the moment he may have felt that she may have played a part in how the shooter(s) found him. And two, he being someone who works in the assassination game most certainly can't have the police involved until he's left the region. While he himself told her to call the police it was probably just the excuse he needed to get her moving.
This scene goes a long way in conveying just how little trust someone in Jack's line of work can have in virtually anyone he meets.
