Firstly, I totally love this plugin so I gave you 5 star. I have enabled litespeed images lazy load. Is it the reason why the images dont have alt tag.
There are more than 16 images which shows no alt tags for say one of them is wp-content/uploads/2021/03/IMG_20180211_20122411-48×48.jpg” class=”avatar avatar-34 photo” alt=”” loading=”lazy”Forum: Fixing WordPress

Will try to disable the tag manager and as I said earlier sometimes or negligible times it slows down other than that it works perfectly.Thank you for making such an important plugin for bloggers☺️
