We tested this behavior with different SMTP configurations and other forms, and the Reply-To address works correctly in those cases. However, the issue only occurs when using a Forminator form.
Based on our testing, this appears to be related to how Forminator handles the dynamic Reply-To address. We recommend reaching out to the Forminator support team so they can review how the Reply-To value is being passed from the form.
I just tested the form after disabling your plugin – so it uses the default WordPress sending. It sends the mail correctly, with the reply-to address as setup in Forminator

Thank you for your patience.
Our team has reviewed this, and the setup should work correctly with the following configuration:
- Post SMTP → Settings → Message → Reply-To: Leave this field empty.
- Forminator → Email Notification → Advanced → Reply-To: Set this to the submitter’s email field (for example,
{email-1}), without any extra spaces.
You can also check the Post SMTP → Email Log to confirm whether the Reply-To header is showing the submitter’s email address or webmaster@.
Could you please check these settings on your site and let us know if you have already configured them this way? If so, please share what you currently have set in both Reply-To fields so we can review further.
