Update build to use octopress-hooks

Guys I just wanted to ask you to update docs / _plugins / process_toc.rb with

require "octopress-hooks" which replaces require "jekyll-page-hooks"

otherwise your grunt task will not create the md files as expected.

It appears that we ran into a problem with the latest version of Jekyll, 2.4.0.

The current system works with Jekyll 2.1.1. We'll probably need to update some files, but in the meantime, we should document the version requirement.

Right, installing https://rubygems.org/gems/jekyll-page-hooks does not help.

All html files in _site/ directory are getting the same error (jekyll does not process th md file correctly!) and the main content is not shown on the web page.

Right now, you need Jekyll 2.1, which works with jekyll-page-hooks.

I imagine we should update, but we'll need to test everything with octopress-hooks and the newer Jekyll.

In the meantime, I've updated the README to describe the limitation.
