One drug, 67 PICOs. The evidence map your BRA can't afford to miss A new peer-reviewed analysis just quantified something the JCA community has been sensing for years: PICO scoping under the EU HTA Regulation is far more complex than most HTDs are prepared for. Key findings from 35 multi-country exercises: -Average 8 consolidated PICOs per indication -A single NSCLC asset generates up to 67 PICOs, driven by biomarker cut-offs and histology sub-populations -Most published exercises covered only 3–10 countries; JCA involves all 27 -HTDs have 100 days to finalize the dossier once scoping closes -Consolidation criteria remain opaque, no clear published thresholds for which PICOs get merged or dropped Evidence generation planning needs to start much earlier than most teams realize. Across the industry, teams are constantly looking to make high-stakes decisions such as which drug to advance, what trial to redesign and which signal to trust, all based on fragmented, partially integrated evidence. By the end of 2026, 80% of pharma companies are expected to rely on AI for benefit–risk evaluation. This is a literal shift in infrastructure. If an AI system contributes to a medical decision, who is responsible? The clinician? The institution? The developer? The regulator? As AI becomes more embedded in drug development and clinical strategy, this question is no longer theoretical. AI does more than generate insights: it influences decisions, sometimes at scale. In healthcare, decisions have consequences: • For patients • For clinical outcomes, and • For entire development programs. Rather than rejecting AI, we have to avoid systems where: • Reasoning is opaque, • Assumptions are hidden, and • Accountability is unclear. At ArcaScience, accountability is a core design principle. In drug development, we often talk about “evidence.” But what does good evidence actually mean? Today, decision-making relies on a patchwork of sources : • Clinical trial data • Scientific publications • Real-world evidence • Safety reports • Mechanistic biology While each of these is valuable, none of them, on their own, is sufficient.
