About Upheld
Upheld is clinical revenue integrity for home health. Every episode gets a readiness score, capture dollars you can still add, and protect dollars at risk — LUPA, takebacks, documentation gaps, and undercoding. Anchored to CMS period payment. Not another QA checklist.
Upload a sample or de-identified chart. No EMR install. In minutes you get a plain-language report: readiness 0–100, dollar estimates, and a ranked fix list with suggested corrections and CMS references. Print, share, export CSV, or email.
Checks run in one pass across clinical documentation, compliance basics (face-to-face, homebound, signatures, certification timing), and revenue risk. AI suggests; your team decides. Not an EMR, not coding outsourcing, not a certified grouper.
Start with a free chart scan — no credit card. Then save findings, work a list, and batch retrospective reviews. Prefer de-identified charts until a BAA is in place.
Capture what you earned. Protect what you bill now.

Solid scan, pricing jump is steep
The free Chart Scan is genuinely useful: I pasted a sample home health chart and got a readable report on submission readiness with the dollar impact split out, and the CMS references made it easy to defend findings to our billing lead. The gap between free and the $1,442 tier is the real sticking point, and I couldn't find anything in the docs about an API or an EHR/EMR integration, so for now it's manual upload or copy-paste per chart, which won't scale past a handful of reviews a week. The report itself is clear enough that a non-clinical person can follow it, which I didn't expect. I'd like to see usage limits spelled out on the free plan and some kind of export or webhook so the results can land in our reporting stack.
