How do you help an AI-assisted security reviewer understand an entire codebase when there’s way too much information to fit within an LLM’s context window? And how do you get relatively consistent output from inherently inconsistent tools? Those were two of the challenges we faced as we scaled SAGE, our AI-assisted security reviewer. In Part 2 of our series, we share how context compression and a deterministic approach to change detection helped SAGE move from reviewing diffs in isolation to reasoning about the code around them. Today, Off-by-1 Labs, 1Password’s security research team, is releasing new research into what happens when AI is asked not just to find vulnerabilities, but to fix them. Across six recently disclosed, complex vulnerabilities, we generated 6,480 patches using two frontier AI models. The results reveal a critical gap between generating a patch and producing one that is actually safe: ▶️ Just 26.0% fully resolved the vulnerability without materially changing application behavior ▶️ 20.1% fixed the vulnerability but changed how the application worked ▶️ 53.9% failed to resolve the vulnerability, introduced a new vulnerability, or did both We call them FLAWED: Fix-Like Artifacts with Embedded Defects. As AI accelerates vulnerability discovery, defenders need equally rigorous ways to verify the patches AI produces. Our research shows that expert human review and execution-grounded testing remain essential. We’re releasing the full research paper, dataset, and open-source tooling to help security teams evaluate AI-generated patching against their own codebases.
